Teeth Grinding: Causes, Consequences and What to Do

Waking up with jaw pain, noticing that your teeth feel more sensitive in the morning, or being told by someone that you grind your teeth during the night are common complaints in dental appointments. Bruxism, the clinical term for this habit, is more common than many people realise and can have a genuine impact on oral health and quality of life.


What Is Bruxism?


Bruxism is the involuntary grinding or clenching of the teeth. It may occur during sleep, known as sleep bruxism, or while awake, known as awake bruxism, which is generally associated with clenching the teeth without movement.


In many cases, people are not even aware that they do it. They may be alerted by a family member or by their dentist, who identifies signs of tooth wear during a routine appointment.


Although it is more commonly associated with adults, bruxism can occur at any age. It is not always linked to a single cause, as emotional, functional and behavioural factors often combine.


Main Causes


Stress and Anxiety


Stress is currently considered one of the main factors associated with bruxism, particularly awake bruxism. Periods of greater emotional tension or anxiety often coincide with a worsening of the habit.


Occlusal Imbalances


An improper contact between the upper and lower teeth may contribute to the development or worsening of bruxism, as the masticatory system unconsciously attempts to adjust this relationship.


In these cases, premature contact between two specific teeth may be enough to trigger a pattern of chronic clenching, even when the person is unaware of the habit.


Sleep-Related Factors


Sleep bruxism is also associated with changes in sleep patterns, including brief awakenings and, in some cases, sleep-related breathing disorders. The consumption of caffeine and alcohol, as well as certain medicines, may also influence how frequently it occurs.


In adults, this association is particularly relevant. In some cases, sleep bruxism may be a response to minor airway obstructions during sleep. Complaints such as snoring or unrefreshing sleep should therefore not be ignored when they occur alongside bruxism.


Bruxism in Children


Bruxism is also common in children, particularly during tooth eruption and the first years of life. In most cases, it tends to be temporary and resolves naturally as the masticatory system develops.


Even so, it should be monitored by a dentist, as significant tooth wear or complaints of pain may justify a more detailed assessment, including the relationship between bruxism, dental occlusion and habits such as mouth breathing.


Consequences for the Teeth and TMJ


Tooth Wear


Repeated and excessive contact between the teeth causes enamel wear. In more advanced cases, this may expose the dentine and increase tooth sensitivity.


Temporomandibular Joint Pain


The pressure exerted during bruxism places additional strain on the temporomandibular joint, or TMJ, and the muscles involved in chewing. This may lead to pain, restricted mouth opening or clicking and other joint noises.


When TMJ involvement is suspected, the diagnosis should always include cone beam computed tomography, known as CBCT, which allows the joint and surrounding bone structures to be assessed accurately.


Headaches and Muscle Tension


People with bruxism frequently report headaches, particularly upon waking, caused by prolonged tension in the muscles of the face and neck during the night.

These headaches are typically located around the temples and may sometimes be mistaken for migraines, delaying the correct diagnosis of their cause.



How Is Bruxism Diagnosed?


In most cases, bruxism is diagnosed through a clinical assessment. The dental professional examines the pattern of tooth wear, evaluates the facial muscles and asks about symptoms such as pain upon waking or tooth sensitivity.


When there are signs of TMJ involvement, the assessment may be complemented by imaging examinations such as CBCT. When an associated sleep-related breathing disorder is suspected, a sleep study may also be recommended in collaboration with other medical specialties.


What to Do: Treatment Options


Dental Gutter


A dental gutter is currently one of the most commonly used solutions for protecting the teeth from the wear caused by sleep bruxism.

Stress Management


As stress is a significant factor, particularly in awake bruxism, emotional management strategies, relaxation techniques or, when necessary, specialist support may help reduce the frequency of the habit.


In some cases, simply becoming aware of the habit and taking short, conscious breaks throughout the day to relax the facial muscles may be enough to reduce clenching while awake.


Correcting Occlusal Imbalances, When Appropriate


When there is a clear relationship between dental occlusion and bruxism, orthodontic treatment may be considered to rebalance the contact between the teeth.

This decision is always made on a case-by-case basis following a detailed clinical assessment.


Other Treatment Options


In more complex situations, bruxism treatment may require a multidisciplinary approach. This may include orofacial physiotherapy to reduce muscle tension or other therapeutic options considered in conjunction with the treating clinician.


The choice of treatment always depends on the identified cause and the severity of the symptoms. There is no single solution that is suitable for everyone.


When to Seek Help


If you regularly wake up with a painful jaw, notice unusual wear on your teeth or experience frequent headaches at the beginning of the day, it is advisable to arrange an appointment.


Identifying the cause of bruxism and adopting the appropriate solution, whether this involves a dental gutter or stress management, can help protect the teeth and the temporomandibular joint in the long term.


An accurate diagnosis allows treatment to address the underlying cause of bruxism, avoiding generic approaches that may not provide the expected relief.
